Brian E. Hill, MD is a physician, healthcare system architect, and the co-founder and CEO of HIPnation, an innovative healthcare ecosystem designed to improve care quality while materially reducing cost and expanding access. His work is grounded in systems thinking—shaped by decades of clinical practice, deep engagement with healthcare economics and policy, and more than ten years designing and operating a real-world alternative to the traditional healthcare model.

Dr. Hill began his career as a practicing urologist, where he experienced firsthand how misaligned incentives, fragmented payment structures, and administrative complexity distort clinical decision-making and drive unnecessary cost. In daily practice, he observed that healthcare was optimized for billing and intermediaries rather than outcomes, access, or long-term health. These experiences led him to view healthcare not as a clinical failure, but as a system design problem.

To understand that system more fully, Dr. Hill immersed himself in healthcare economics, financing mechanisms, and policy. He worked in the healthcare policy space, traveled nationally speaking on healthcare economics and delivery models, and authored Stop the Noise: A Physician’s Quest to Silence the Politics of Health Care Reform, which examined why reform efforts repeatedly fail when incentives, information flow, and accountability are misaligned.

Throughout this period, Dr. Hill has remained clinically active, intentionally maintaining patient relationships to stay grounded in the core unit of healthcare: the patient and the physician working together. He also continues to serve in physician leadership roles, sitting on the boards of both the Medical Association of Georgia and the Medical Association of Atlanta, providing ongoing insight into how system design decisions affect physicians across specialties and practice environments. Earlier, he served on the founding board of Emory Healthcare Network’s clinically integrated network, where he observed large-scale attempts to align cost, quality, and utilization.

In 2015, Dr. Hill founded HIPnation to move from analysis to execution. Over the past decade, he has led the development of a fully integrated healthcare ecosystem—intentionally redesigning incentives across primary care, specialty care, pharmacy, and employer financing, while delivering care inside the model itself. This has enabled continuous learning, feedback, and iteration based on real utilization, outcomes, and human behavior.

Today, HIPnation functions as a coherent system rather than a collection of point solutions—lowering cost, improving access, and restoring the patient and physician experience. Dr. Hill speaks from the perspective of a clinician-operator who has studied the system, built within it, and redesigned it in practice.
